In this publication, the review of recent publications on transitional care models helped to identify existing interventions that align with the new CMS Age-Friendly Hospital Measure in a current state analysis, highlighting transitional care interventions that align with at least one of the measure domains, identify gaps to fill, and summarize outcomes to give health systems guidance on ways they might design transitional care programs that align with the CMS Age-Friendly Hospital Measure.
