Dartmouth Health Recognized by IHI as Level 2 using the Medicare Annual Wellness visit as the Gateway to the 4Ms in ambulatory care. Moving forward with full system implementation across all sites Inpatient, Ambulatory, Emergency Department, Home Health, Rural Health Clinic One of the most rural academic medical centers in the country with 6 partner hospitals (4 critical access hospitals). Ambulatory care at each site Learn from the network specifically how demonstrate specific outcomes that are a priority for our system including reducing LOS and reducing unwarranted and unnecessary care.
