- The John A. Hartford Foundation is excited to share the New Partners Program Call for Proposals that marks an important new chapter in The John A. Hartford Foundation’s work to improve care for older adults. Through this opportunity, they are inviting new organizations, leaders and ideas into our community of grantees and partners.
- They are seeking fresh perspectives and innovative, evidence-informed approaches designed to have a measurable impact in at least one of the three priority areas:
- Age-Friendly Health Systems
- Family Caregiving
- Serious Illness and End-of-Life Care.
- US-based 501(c)(3) organizations that have not received a major JAHF grant are invited to propose national or multi-site initiatives.
- Up to 10 grants will be awarded, each for up to $300,000 over two years.
- Grant portal registration is due Oct. 21 at noon ET, and letters of intent are due Oct. 27 at noon ET.
