Northwell Health has launched the Advancing Age-Friendly Care Collaborative, an experiential consulting and training program —and we’re now accepting applications for U.S. hospitals to participate. Funded by The John A. Hartford Foundation, this national collaborative will guide selected hospitals in advancing the Institute for Healthcare Improvement (IHI) Age-Friendly Health System (AFHS) 4Ms Framework, with a special focus on patient experience outcomes in partnership with Press Ganey.
Selected participants will receive $13,300 in value and engage in a 15-month immersive experience designed to drive measurable improvements in patient experience.
